Introducción: La infección por Fasciola hepatica es una enfermedad zoonótica de distribución mundial, desatendida y subdiagnosticada. Objetivo: Determinar la frecuencia de Fasciola hepatica en una población preescolar en Tartar Chico, distrito de Baños del Inca, en la región Cajamarca. Métodos: Estudio transversal en 48 niños de una institución educativa inicial. Los padres entregaron 3 muestras de heces para el estudio parasitológico seriado y completaron una encuesta epidemiológica. La identificación de F. hepatica y otros parásitos se realizó con las pruebas de sedimentación rápida de Lumbreras, examen directo y Kato-Katz. Para describir usamos frecuencias y porcentajes, para el análisis bivariado aplicamos Chi-cuadrado o prueba exacta de Fisher. Resultados: La frecuencia de Fasciola hepatica fue 4,17%. Además, estimamos una proporción de 8,33% para Ascaris lumbricoides, 4,17% de Diphyllobothrium pacificum y 2,08% de uncinarias; así como parásitos contaminantes Entamoeba coli, Blastocystis hominis. Conclusión: Encontramos una frecuencia de 4,17% de fascioliasis entre preescolares de una comunidad altoandina del Perú.


Introduction: Fasciola hepatica infection is a globally distributed, neglected and underdiagnosed zoonotic disease. Objectives: To determine the frequency of Fasciola hepatica infection among a preschool population in Tartar Chico, Baños del Inca, Cajamarca. Methods: Cross-sectional study in 48 children of an initial educational institution. Parents delivered 3 stool samples for the serial parasitological study and completed an epidemiological survey. The identification of F. hepatica and other parasites was carried out with the Lumbreras rapid sedimentation tests, direct examination, and Kato-Katz. For descriptive analysis, frequency and percentages were used, for the bivariate analysis, Chi-square or Fisher's exact test was used. Results: The frequency of F. hepatica was 4,17%. In addition, a proportion of 8,33% of Ascaris lumbricoides, 4,17% of Diphyllobothrium pacificum and 2,08% of hookworms; as well as contaminating parasites Entamoeba coli, Blastocystis hominis. Conclusions: A frequency of 4,17% of fascioliasis was found among preschoolers from a high Andean community in Peru.

Introduction: Human fascioliasis is a plant-borne and water-borne infection caused by the trematodes Fasciola hepatica and Fasciola gigantica. It is one of the main neglected tropical diseases, and infections in humans occur via the ingestion of contaminated water and food. This study reviews all the recorded cases of human fascioliasis in Brazil under different climatic conditions in the national territory. Methodology:A survey of human fascioliasis cases in Brazil was carried out using the Google Scholar, Lilacs and PubMed databases. The climatic variables such as temperature, precipitation, moisture and altitude were obtained from the database of the Instituto Nacional de Meteorologia (INMET). Results: Between the years 1958 and 2022, sixty-six cases of human fascioliasis were recorded in places with temperature levels between 22 °C to 33 °C, humidity 78% to 86%, precipitation 90 mm to 167 mm, and at an altitude of 16 to 935 meters above sea level. Conclusion: The parasite's ability to adapt to different climatic conditions is observed in Brazil and the number of cases of human fascioliasis in the national territory may be higher due to underreporting related to the difficulty in diagnosing the infection.


Introdução: A fasciolíase humana é uma infecção de origem vegetal e hídrica, causada pelos trematódeos Fasciola hepatica e Fasciola gigantica. É uma das principais doenças tropicais negligenciadas, e as infecções em humanos ocorrem através da ingestão de água e alimentos contaminados. Este estudo revisa todos os casos registrados de fasciolíase humana no Brasil sob diferentes condições climáticas no território nacional. Metodologia: Um levantamento dos casos de fasciolíase humana no Brasil foi realizado nas bases de dados Google Scholar, Lilacs e PubMed. As variáveis climáticas como temperatura, precipitação, umidade e altitude foram obtidas do banco de dados do Instituto Nacional de Meteorologia (INMET). Resultados: Entre os anos de 1958 e 2022, sessenta e seis casos de fasciolíase humana foram registrados em locais com níveis de temperatura entre 22 °C a 33 °C, umidade de 78% a 86%, precipitação de 90 mm a 167 mm e altitude de 16 a 935 metros acima do nível do mar. Conclusão: A capacidade de adaptação do parasito a diferentes condições climáticas é observada no Brasil e o número de casos de fasciolíase humana no território nacional pode ser maior devido à subnotificação relacionada à dificuldade de diagnóstico da infecção.

Es una de las principales enfermedades tropicales desatendidas, y las infecciones en humanos ocurren a través de la ingestión de agua y alimentos contaminados. Este estudio revisa todos los casos registrados de fascioliasis humana en Brasil bajo diferentes condiciones climáticas en el territorio nacional. Metodología: Se realizó una encuesta de casos de fascioliasis humana en Brasil utilizando las bases de datos Google Scholar, Lilacs y PubMed. Las variables climáticas como temperatura, precipitación, humedad y altitud se obtuvieron de la base de datos del Instituto Nacional de Meteorología (INMET). Resultados: Entre los años 1958 y 2022 se registraron sesenta y seis casos de fascioliasis humana en lugares con temperatura entre 22 °C a 33 °C, humedad entre 78% y 86%, precipitación entre 90 mm y 167 mm y una altitud de 16 a 935 metros sobre el nivel del mar. Conclusión: La capacidad de adaptación del parásito a diferentes condiciones climáticas se observa en Brasil y el número de casos de fascioliasis humana en el territorio nacional puede ser mayor debido al subregistro relacionado con la dificultad en el diagnóstico de la infección.

RESUMEN En muchos países la fasciolosis y la paramfistomosis representan un grave problema para la salud del sector pecuario. En Colombia hay registros de ambas distomatosis en bovinos, la mayoría en el trópico alto andino, las cuales generan pérdidas econômicas anuales cercanas a 40 000 000 000 COP. El objetivo de esta investigación fue determinar la prevalencia de fasciolosis y paramfistomosis en vacunos de la hacienda La Candelaria, Caucasia (Colombia), y la presencia de caracoles hospederos intermediarios. Para cumplirlo, se realizó un estudio descriptivo de corte transversal con muestreo no probabilístico por conveniencia. Se recolectaron heces de los animales y se hizo el diagnóstico mediante la técnica modificada de Dennis. Se establecieron las prevalencias de los digeneos de acuerdo con el sexo, peso, edad y raza. Se recolectaron caracoles dulciacuícolas en la zona estudiada y se identificaron por morfologia. Se analizaron 466 muestras fecales de 178 bovinos, de las razas Cebú (Bos indicus), BON (blanco orejinegro) y del cruce entre ellas. Se diagnosticaron F. hepatica y Paramphistomidae con prevalencias del 2,2% y 30,9%, respectivamente. En el 1,1% de los vacunos se diagnosticó coinfección. Ambas trematodosis prevalecieron en las hembras (p = 0,03). Se identificaron moluscos dulciacuícolas Ampullariidae, Physidae y Planorbidae sin estadios larvarios de digeneos. Se concluyó que los bovinos de doble propósito de la hacienda La Candelaria están expuestos a F. hepatica y Paramphistomidae, y, probablemente, se infectan en los predios de la hacienda. Paramphistomidae es más prevalente que F. hepatica, lo cual concuerda con lo descrito en algunos estudios realizados en hatos del trópico alto andino colombiano. Paramphistomidae se encontró en todos los grupos etarios.


ABSTRACT Fasciolosis and paramphistomosis are a major health problem for the livestock economy worldwide. In Colombia, both distomatosis are reported in cattle, particularly in high Andean tropics, with annual economic losses close to COP 40 billion. The goal of this study was to determine the prevalence of fasciolosis and paramphistomosis in cattle from La Candelaria farm, Caucasia (Colombia), and the presence of intermediate host snails. A descriptive cross-sectional study was carried out with non-probability convenience sampling. Stool samples were collected from the animals and the diagnosis was made using the modified Dennis technique. Digenea prevalence were determined according to sex, weight, age, and race. Freshwater snails were collected in the studied area and were identified by morphology. 400 and 66 fecal samples from 178 bovines of the Zebu (Bos indicus), BON (white-eared white) breeds and the cross between them were analyzed. Fasciola hepatica and Paramphistomidae were diagnosed with a prevalence of 2,2% and 30,9%, respectively. Coinfection was diagnosed in 1,1% of the cattle. Both trematodosis were most frequent in females (p = 0,03). Freshwater molluscs Ampullariidae, Physidae and Planorbidae without digenea larval stages were identified. It was concluded that dual-purpose cattle from La Candelaria farm are exposed to F. hepatica and Paramphistomidae and are probably infected on the farm grounds. Paramphistomidae was more prevalent than F. hepatica, which agrees with other studies in herds from the Colombian high Andean tropics. Paramphistomidae was found in all age groups.

ABSTRACT

Objetivo : Determinar el rendimiento diagnóstico de la técnica de inmnunocromatografía o flujo lateral para la detección de anticuerpos en pacientes con Fasciolosis humana. Materiales y métodos : Estudio observacional, prospectivo y de corte transversal. Hemos desarrollado una prueba de flujo lateral (Fasciorap) para el diagnóstico serológico de las Fasciolosis humana por Fasciola hepatica, compuesta por antígenos de excreción-secreción de formas adultas conjugadas con orocoloidal de 40 nm y una proteína A e IgG de conejo anti Fasciola hepatica como reactivos detectores en la línea de prueba y control, flanqueados por almohadillas en un cassette. Se evaluaron 240 sueros, 120 positivos, 50 sueros de pacientes con otras parasitosis, 20 de pacientes con enfermedades infecciosas y 50 sueros de personas no parasitadas, la interpretación de resultados se realizó por inspección visual a los 15 minutos de aplicada las muestras. Resultados : La prueba detectó la presencia de anticuerpos en el suero de pacientes con fasciolosis, alcanzando una sensibilidad de 92,5%, una especificidad de 94,17%, un valor predictivo positivo de 94,07% y negativo de 92,62%; con 100% de concordancia en la repetibilidad y reproducibilidad. Conclusiones : Fasciorap detecta casos de fasciolosis, por lo tanto, es una potencial prueba diagnóstica en zonas endémicas donde se requiere pruebas de punto de atención


Objective : To determine the diagnostic performance of the immunochromatography technique or lateral flow for the detection of antibodies in patients with human fasciolosis. Materials and methods : Observational, prospective and cross-sectional study. We have developed a lateral flow test (Fasciorap) for the serological diagnosis of fasciolosis due to F. hepatica, composed of excretion-secretion antigens of adult forms conjugated with orocolloid of 40 nm and a protein A and IgG of rabbit anti F. hepatica as detector reagents in the test and control line, flanked by pads in a cassette. 240 sera were evaluated, 120 positive, 50 sera from patients with other parasites, 20 from patients with infectious diseases and 50 sera from non-parasitized people, the interpretation of results was performed by visual inspection 15 minutes after applying the samples. Results : The test detected the presence of antibodies in the serum of patients with fasciolosis, reaching a sensitivity of 92.5%, a specificity of 94.17%, a positive predictive value of 94.07% and a negative predictive value of 92.62%; with 100% agreement on repeatability and reproducibility. Conclusions : Fasciorap detects cases of fascioliasis, therefore, it is a potential diagnostic test in endemic areas where point-of-care testing is required

ABSTRACT

As síndromes consumptivas possuem ampla variedade de diagnósticos diferenciais, sendo um desafio na prática médica. Neste relato, a paciente apresentou astenia, diarreia e tosse, evoluindo com dor em hipocôndrio direito. A elucidação adveio da análise histopatológica após a primeira suspeição ser de lesão hepática maligna. Como desfecho, a infecção pelo trematódeo Fasciola hepatica, endêmico de clima temperado, foi a causa etiopatogênica. Nesse sentido, a intenção deste relato é trazer para discussão diagnósticos diferenciais de síndrome consumptiva tendo em vista etiologias endêmicas.


Consumptive syndromes have an ample variety of differential diagnoses and are a challenge in the medical practice. In this report, the patient presented asthenia, diarrhea, and cough, evolving to pain in the right hypochondrium. The elucidation came from the histopathological analysis after the first suspicion of it being a malignant hepatic lesion. The outcome, infection by the trematode Fasciola hepatica, endemic of temperate climate, was the etiopathogenic cause. In this sense, this report aims to discuss differential diagnostics of consumptive syndrome considering endemic etiologies.

ABSTRACT

Resumen Debido al uso frecuente del triclabendazol (TCBZ) por parte de los productores, sumado al reporte reciente de resistencia de Fasciolahepatica al fármaco detectada en rodeos y majadas de Neuquén, se llevó a cabo el presente trabajo con el objetivo de evaluar la eficacia del TCBZ como fasciolicida en caprinos bajo un sistema de producción trashumante del norte de la provincia de Neuquén, Argentina. La eficacia se evaluó mediante un test de reducción de conteo de huevos de F. hepatica en caprinos Criollo Neuquino parasitados naturalmente. Se utilizaron dos grupos de animales: un Grupo Control (n: 10) que no recibió tratamiento y un Grupo Tratado (n: 10) al que se le administró TCBZ al 10% (Biofasiolex T10-Biogénesis Bagó ®) vía oral a dosis de 10 mg/kg PV. Al día 0 y 21 post tratamiento, se tomaron muestras de materia fecal para análisis coproparasitológicos. Se evidenció una reducción del conteo de huevos de F. hepatica del 100%, indicando el mantenimiento de la eficacia del fasciolicida en los caprinos evaluados. Se discuten los alcances del uso del test de reducción de conteo de huevos en zonas con producción trashumante del ganado.


Abstract Because of the frequent use of triclabendazole TCBZ by farmers and the recent reports of Fasciola hepatica resistance to this drug in herds and flocks from Neuquén, we carried out this study with the aim to evaluate the efficacy of TCBZ as fasciolicide in goats under a transhumance system from northern of Neuquén, Argentina. A fecal egg count reduction test (FECRT) was performed on Creale Neuquino goats naturally parasitized with F. hepatica. Twenty goats were divided into two groups: A control group (n: 10) which was left untreated, and a treatment group (n: 10) that was dosed orally with TCBZ 10% (Biofasiolex T10-Biogénesis Bagó ®). On Day 0 and Day 21 post-treatment, fecal samples were taken and analyzed by coproscopical examination. According to the FECRT, drug efficacy was 100 % and confirms that TCBZ maintains its efficacy against F. hepatica on goats. The usefulness of the FECRT in these zones under transhumance livestock production is discussed.

La Fasciola hepática es una enfermedad frecuente en sierra del Perú. Presentamos un caso de Fasciola hepática procedente de la zona andina de La Libertad, con sintomatología de varios meses de evolución con dolor en hipocondrio derecho, ictericia y coluria. Se realizó TAC abdominal demostrándose una masa hepática probable neoplasia, derivándose a Instituto de Enfermedades Neoplásicas. Se completó los exámenes de laboratorio encontrando eosinofilia y aumentos variables de las pruebas de función hepática. Se planteó como diagnóstico diferencial Fascioliasis y se realizó examen de Western Bloot confirmándose su diagnóstico. Se inició tratamiento con 2 ciclos de Triclabendazol, con evolución clínica favorable.


Hepatic Fasciola is a frequent disease in the Peruvian highlands. We present a case of hepatic Fasciola from the Andean zone of La Libertad, with symptoms of several months of evolution with pain in the right hypochondrium, jaundice and coluria. An abdominal CT scan was performed, demonstrating a liver mass probable neoplasm, and was referred to the Institute of Neoplastic Diseases. Laboratory tests were completed finding eosinophilia and variable increases in liver function tests. Fascioliasis was presented as a differential diagnosis and a Western Bloot examination was performed confirming its diagnosis. Treatment with 2 cycles of Triclabendazole was started, with a favorable clinical evolution.

RESUMEN Este estudio se llevó a cabo para establecer la prevalencia e identificar los factores de riesgo asociados con las infestaciones por trematodos en ganado lechero Holstein en el valle de Paipa, Boyacá. Las muestras fecales recogidas de 100 bovinos seleccionados aleatoriamente se examinaron utilizando una técnica de sedimentación simple para el recuento diferencial de huevos de trematodos. Se buscaron huevos de tres grupos de trematodos: Fasciola hepatica, Paramphistomum spp., y Cotylophorum spp. La prevalencia específica de trematodos para cada grupo fue del 12, 9 y 4%, respectivamente. Se observó una superposición sustancial en el recuento de F. hepatica y Paramphistomum spp. La prevalencia de los tres trematodos identificados en este estudio se asoció significativamente (P < 0,05) con la condición corporal y la raza, mientras que la prevalencia de Fasciola hepatica y Paramphistomum spp., se asoció con la edad. La prevalencia de los tres principales trematodos de importancia para la salud animal con una alta tasa de infestación mixta junto con una mala condición corporal, sugiere una pérdida económica sustancial incurrida debido a la reducción de la productividad del ganado en el área de estudio.


ABSTRACT This study was carried out to establish the prevalence and identify the risk factors associated with trematode infestations in Holstein dairy cattle in the Paipa-Boyacá valley. Fecal samples collected from 100 randomly selected cattle were examined using a simple sedimentation technique for differential trematode egg count. Eggs were sought from three groups of trematodes: Fasciola hepatica, Paramphistomum spp., and Cotylophorum spp. The specific prevalence was 12, 9 and 4%, respectively. Substantial overlap was observed in the count of F. hepatica and Paramphistomum spp. The prevalence of the three flukes identified in this study was significantly associated (P < 0.05) with body condition and race, while the prevalence of Fasciola hepatica and Paramphistomum spp., was associated with age. The prevalence of the three main trematodes of importance for animal health with a high rate of mixed infestation together with a poor body condition, suggests a substantial economic loss incurred due to the reduction of livestock productivity in the study area.

RESUMEN Introducción: la fasciolasis hepática es una enfermedad parasitaria de difícil diagnóstico. La variada y múltiple sintomatología que caracteriza la enfermedad, la expulsión intermitente y tardía de los huevos del parasito, así como el difícil acceso a los estudios inmunológicos en las zonas rurales atenta contra la confirmación de parasitosis en casos esporádicos. En la provincia de Matanzas en los últimos años se constataron enfermos sospechosos de fasciolosis procedentes de Pedro Betancourt con diversos síntomas digestivos, fiebre y eosinofilia. Objetivo: caracterizar los aspectos clínicos y epidemiológicos de los pacientes diagnosticados de fasciolosis hepática. Materiales y Método: se realizó un estudio observacional, descriptivo a pacientes hospitalizados con el diagnóstico presuntivo de fasciolosis hepática en el Servicio de Medicina Interna del Hospital Faustino Pérez en el período enero del año 2011 a diciembre del 2013. La recolección de datos se realizó a través del interrogatorio, examen físico, Historia clínica y encuestas epidemiológicas. Resultados: se muestra la presencia de consumo de plantas acuáticas y consumo de agua sin hervir en los afectados. La fase aguda de la enfermedad en estos pacientes se caracterizó por fiebre, dolor abdominal, hepatomegalia y gran eosinofilia. Conclusiones: el diagnóstico de certeza se realizó a través de pruebas inmunológicas. Esto se debe a que a pesar de los múltiples exámenes parasitológicos de las heces fecales no se identificó huevos. Los pacientes del estudio fueron tratados con el triclabendazol y se logró mejoría clínica de los pacientes y pocas reacciones adversas (AU).


SUMMARY Introduction: the hepatic fasciolosis is a parasitic disease of difficult prognosis. The varied and multiple symptoms characterizing the disease, the intermittent and late ejection of the parasite eggs, and also the difficult access to immunological studies in the country areas sometimes attempts against the confirmation of parasitism. During the last years, in the province of Matanzas, several sick persons from Pedro Betancourt were suspicious of fasciolosis with different digestive symptoms, fever and eosinophilia. Objective: to characterize the clinical and epidemiological aspects of the patients diagnosed of hepatic fasciolosis. Materials and method: an observational, descriptive study was performed in in-patients of the Service of Internal Medicine of the Hospital Faustino Perez with the presumptive diagnosis of hepatic fascilosis in the period January 2011-December 2013. Data collection was made through questioning, physical examination, clinical record and epidemiological inquiries. Results: risk behaviors are evidenced given the water plants and un-boiled water consumption in affected persons. The acute `hase in these individuals is characterized by fever, abdominal pain, hepatomegaly and big eosinophilia. The diagnosis of certainty was made by immunologic tests. The patients were treated with triclabendazol with clinical improvement and little adverse reactions. Conclusions: health professionals when assisting a patient from a country area with eating habits of fresh vegetable consumption and suggestive clinical characteristics should carry out studies to confirm Fasciola hepatica infection (AU).

RESUMEN Introducción: La fascioliasis es una enfermedad causada por el parásito Fasciola hepática, según su cuadro clínico puede clasificarse en dos fases: aguda y crónica, actualmente el diagnóstico se realiza durante la fase crónica, sin embargo, la detección temprana permitiría brindar un tratamiento eficaz y oportuno. Objetivo: Este estudio se realiza con el objetivo de evaluar la validez diagnóstica de las pruebas serológicas para el diagnóstico oportuno de fascioliasis hepática. Materiales y métodos: se realizó una búsqueda sistemática de la literatura en Embase, MedLine, Cochrane Library y LILACS. La selección de estudios se realizó mediante pares de revisores, se seleccionaron estudios de pruebas diagnósticas utilizando: ELISA, ELISA indirecto, ELISA rápido, Fas2 ELISA, Arco2 comparada con técnicas de sedimentación para el diagnóstico de fascioliasis hepática en adultos. Los estudios fueron evaluados mediante la herramienta QUADAS-2. Resultados: Se identificaron siete estudios que responden a la pregunta PICO, con un total de 1317 muestras de suero humano, las pruebas reportaron alta sensibilidad y especificidad, FhTA, ELISA, número de estudios y participantes identificados, estimadores resumen con los correspondientes intervalos de confianza/credibilidad, también se puede discutir el ranking de tratamientos. Discusión: en general la especificidad de las pruebas serológicas fue ligeramente superior que la sensibilidad, existe evidencia de moderada a baja calidad que reporta su rendimiento, sin embargo, la calidad de la evidencia y la heterogeneidad entre los estudios no permite determinar su utilidad durante la fase aguda de la enfermedad. Se necesitan más estudios al respecto. Los estudios fueron clínicamente heterogéneos entre ellos.


ABSTRACT Introduction: Fascioliasis is a disease caused by the parasite Fasciola hepatica, according to its clinical picture, it can be classified into two phases: acute and chronic, currently the diagnosis is made during the chronic phase, however, early detection would allow to provide an effective treatment and timely. Objetive: This study is carried out with the objective of evaluating the diagnostic validity of serological tests for the timely diagnosis of hepatic fascioliasis. Materials and methods: a systematic search of the literature was carried out in Embase, MedLine, Cochrane Library and LILACS. Study selection was performed by pairs of reviewers, diagnostic test studies were selected using: ELISA, indirect ELISA, rapid ELISA, Fas2 ELISA, Arco2 compared to sedimentation techniques for the diagnosis of hepatic fascioliasis in adults. The studies were evaluated using the QUADAS-2 tool. Results: Seven studies were identified that answer the PICO question, with a total of 1,317 human serum samples, the tests reported high sensitivity and specificity, FhTA, ELISA, number of studies and participants identified, summary estimators with the corresponding confidence intervals / credibility, and the ranking of treatments. Discussion: In general, the specificity of the serological tests was slightly higher than the sensitivity, there is evidence of moderate to low quality that reports their performance, however, the quality of the evidence and the heterogeneity between the studies do not allow determining its usefulness during the acute phase of the disease. More studies are needed in this regard. The studies were clinically heterogeneous between them.

Abstract The aim of this research was to assess the copro-prevalence of Fasciola hepatica in owned Chilean breed horses (Equus caballus) residing in the province of Concepción, Chile. The study was carried out throughout October 2017. Samples were taken from all (100%) Chilean breed horse (124 specimens; 45 females and 79 males, aged between 7 months and 24 years old) from the Concepción province. A rectal stool sample was obtained from each animal. This was analyzed using the Army Medical School method (AMS III) technique to detect F. hepatica eggs. Information about the horse's age, sex, previous fasciolicide application, and an assessment of body condition was also obtained. Fisher tests were carried out to analyze the results. Ten horses (8.06%) were positive for the presence of F. hepatica eggs. No significant difference of copro-prevalence was found between age (young 8.47%, old 7.69%), sex (female 11.1%, male 6.33%), the previous application of fasciolicide (dewormed 10%, not dewormed 7.89%), or body condition (low condition 11.1%, high condition 0.7%) categories (Fisher tests: P>0.05 in all cases). The results of this study suggest that Chilean breed horses constitute part of the reservoir of F. hepatica for both ruminant populations and humans in the Concepción province.


Resumo O objetivo desta pesquisa foi avaliar a coproprevalência de Fasciola hepatica em equinos chilenos (Equus caballus), residentes na província de Concepción, região de Bío-Bío, Chile. O estudo foi realizado durante o mês de outubro de 2017. Foram coletadas amostras de todos os equinos estabulados (124 espécimes; 45 fêmeas e 79 machos, com idades entre 7 meses e 24 anos) da província de Concepción. Uma amostra de fezes transretal foi obtida de cada animal, analisada no Laboratório "Dr. Luis Rubilar", da Universidad de Concepción, usando-se a técnica AMS III, para detectar a presença de ovos de F. hepatica. O diagnóstico fecal foi complementado com informações sobre a idade do animal, sexo, aplicação prévia de fasciolicida e uma avaliação da condição corporal. Testes de Fisher foram realizados para analisar os resultados. Dos 124 equinos, 10 (8,06%) foram positivos para ovos de F. hepatica. Não foi encontrada associação significativa entre idade, sexo, aplicação prévia de fasciolicida ou condição corporal com a presença de ovos. Um cavalo tratado com triclabendazol foi positivo para ovos de F. hepatica.

A cross sectional study was conducted to determine the prevalence and economic significance of bovine fasciolosis at Gondar Elfora abattoir, Gondar Ethiopia from November 2018 to April 2019. A total of 384 cattle were selected by systematic random sampling techniques for postmortem examination at Gondar Elfora abattoir. Out of these 90 (23.4%) cattle were found positive for fasciolosis. The prevalence of fasciolosis in poor body conditioned cattle were known to be 30.8% followed by medium body conditioned (22.7%) and good conditioned animal (15.2%) . Statistical analysis of the data showed the presence of significant difference (P=0.028) on the prevalence of fasciolosis on the basis of body condition score. Higher prevalence of bovine fasciolosis in the study area was observed in older than adult animals 28.7% and 19.5% respectively. There was statistically significant association among age of the animal and prevalence of bovine fasciolosis (P=0.037). The predominant Fasciola species identified was Fasciola hepatica 57 (14.8%) followed by Fasciola gigantica 21 (5.5%) and mixed species (Fasciola hepatica and F. gigantica) 12 (3.1%). The total annual economic loss due to bovine fasciolosis in Gondar Elfora abattoir was calculated as 1,919,640 ETB (65,922 USD). The economic loss due to bovine fasciolosis at the study area showed that fasciolosis is an economically important disease in the Gondar Elfora abattoir. Hence, a combination of anthelminthic and vector control measures have to be used to ensure a satisfactory degree of control in the long run

El diagnóstico de fasciolosis humana, enfermedad zoonótica causada por el parásito Fasciola hepatica reúne los resultados de las técnicas: concentración por sedimentación (copa-cónica), FasciDIG en heces y FasciDIG en suero, además de los criterios clínico-epidemiológicos. FasciDIG constituye un ensayo inmunoenzimático que detecta antígenos de excreción-secreción de F. hepatica a partir de muestra de suero y heces. Permite diagnosticar la infección en cada una de las formas clínicas de la enfermedad y presenta una sensibilidad diagnóstica superior a las técnicas convencionales que detectan huevos del parásito (copa-cónica), por lo que se consideró oportuno abordar algunos conceptos relacionados con esta técnica inmunodiagnóstica y analizar su aplicabilidad para el diagnóstico oportuno y eficaz de esta parasitosis(AU)


Diagnosis of human fasciolosis, a zoonotic disease caused by the parasite Fasciola hepatica, combines the results of the following techniques: conical cup, feces FasciDIG and serum FasciDIG, as well as clinical-epidemiological criteria. FasciDIG is an enzyme immunoassay that detects F. hepatica excretion / secretion antigens in serum and feces samples. It makes it possible to diagnose infection at each of the clinical stages of the disease with a higher diagnostic sensitivity than conical cup. Therefore, it was considered appropriate to address a number of concepts regarding this immunodiagnostic technique and analyze its applicability in the timely and effective diagnosis of this helminth infection(AU)

Introducción: en el paisaje epidemiológico de la fascioliasis humana, dos aspectos son de fundamental importancia: a) presencia de hospederos definitivos (especialmente ganado bovino y humanos) parasitados con adultos del tremátodo y; b) presencia de hospederos intermediarios invertebrados, moluscos de la familia Lymnaeidae, en los que se desarrollan los estadios larvarios, además de plantas acuáticas de consumo humano. Material y métodos: con el objetivo de prevenir brotes de fascioliasis humana mediante la correcta identificación de aspectos de interés epidemiológico, un humedal fue seleccionado en el sector de Palo Blanco, municipio de Jarabacoa, provincia de La Vega, en el cual se realizó un estudio malacológico en octubre de 2016, en busca de especies de moluscos limneidos e hidrófitos asociados. En el laboratorio se obtuvieron los índices de infección natural y experimental a Fasciola hepatica. Resultados: se identificó el limneido Pseudosuccinea columella, en proporciones de 250 especímenes por persona/hora. El vegetal predominante fue el berro (Nasturtiun officinale), cultivado comercialmente en tres cuadrantes del humedal y en el otro cuadrante se observaron bovinos pastando. El índice de infección experimental de los moluscos fue de 94 % a la cepa simpátrica de F. hepatica de Dajabón; no se encontraron moluscos parasitados naturalmente. Conclusiones: Pseudosuccinea columella debe considerarse como un molusco de relevancia epidemiológica en la transmisión de la fascioliasis en República Dominicana, a tenor de su dispersión y elevado índice de infección experimental obtenido en el presente estudio


Introduction: in the epidemiological landscape of human fascioliasis, two aspects are of fundamental importance: a) presence of definite hosts (especially cattle and humans) parasitized with trematode adults and b) presence of invertebrate intermediate hosts, mollusks of the family Lymnaeidae, in which larval stages develop, in addition to aquatic plants for human consumption. Material and methods: in order to prevent outbreaks of human fascioliasis by correctly identifying aspects of epidemiological interest, a wetland was selected in the sector of Palo Blanco, municipality of Jarabacoa, province of La Vega, in which a malacological study was carried out in October 2016, in search of limneid mollusk species and associated hydrophytes. Natural and experimental infection rates to Fasciola hepaticawere obtained in the laboratory. Results: the lymnaeid Pseudosuccinea columella was identified, in proportions of 250 specimens collected per person/hour. The predominant vegetable was the watercress (Nasturtiun officinale), commercially cultivated in three quadrants of the wetland and in the other quadrant were observed cattle grazing. The rate of experimental infection of mollusks was 94 % to the sympatric strain of F. hepatica of Dajabón; no naturally parasitized mollusks were found. Conclusions: Pseudosuccinea columella should be considered as a mollusk of epidemiological relevance in the transmission of fascioliasis in the Dominican Republic, due to its dispersion and high rate of experimental infection obtained in the present study

Abstract In South America, fascioliasis caused by the trematode Fasciola hepatica is an anthropozoonosis disease associated with significant economic losses and poor animal welfare. The objective of this study was to determine the prevalence of F. hepatica in the liver of buffaloes slaughtered from 2003 to 2017 in Brazil, and to perform a forecast analysis of the disease for the next five years using the Autoregressive Integrated Moving Average (ARIMA) model. Data analysis revealed an incidence of 7,187 cases out of 226,561 individuals. The disease presented a considerable interannual variation (p<0.005). Fasciola hepatica was more prevalent in the southern states of Brazil; Paraná, Rio Grande do Sul, and Santa Catarina, presenting 11.9, 7.7, and 3.2% of infected livers, respectively. The high frequency of liver condemnation in Paraná was influenced by weather conditions. The ARIMA models calculated a constant trend of the disease, depicting an average of its future prevalence. The models also described a worse-case and a positive-case scenario, calculating the effects of intervention measurements. In reality, there is an urgent need for regular diagnostic in the animals (fecal and immune diagnose) and in the environment (intermediate host), in order to avoid the high rates of infection.


Resumo Na América do Sul, a fasciolose causada pelo Trematoda Fasciola hepatica é uma antropozoonose associada a perdas econômicas significativas e baixo grau de bem-estar animal. O objetivo deste estudo foi determinar a prevalência de F. hepatica no fígado de búfalos abatidos entre 2003 a 2017 e realizar uma análise de previsão da doença para os próximos cinco anos, utilizando o modelo Auto-Regressivo Integrado de Médias Móveis (ARIMA). A análise dos dados revelou uma incidência total de 7.187 casos em 226.561 indivíduos. Houve um acentuado grau de variação interanual nas taxas de prevalência (p<0,005). Fasciola hepatica foi mais prevalente nos estados do sul do Brasil; Paraná, Rio Grande do Sul e Santa Catarina, com 11,9; 7,7; e 3,2% de fígados condenados, respectivamente. A alta incidência de condenação de fígado no Paraná foi influenciada pelo fator climático. Os modelos ARIMA indicaram uma tendência constante na ocorrência da doença, destacando um padrão futuro da doença. Os modelos também descreveram cenários de piora e de possível melhoria, calculando os efeitos de medidas de intervenção. Assim, existe a urgência de realizar diagnóstico constante nos animais (coprológico e diagnóstico imunológico) e no ambiente, para que se evite os altos índices de infecção.

ABSTRACT

Abstract Fasciola hepatica is a parasite introduced in America from Europe, being considered of concern for public health and animal production, especially in countries with endemic areas as occur in Chile. In this country, the research has been focused mainly in domestic ruminants, with native and introduced wildlife being largely ignored. Thus, the aim of this study was to report the infection by Fasciola hepatica in an introduced mammal, the European hare, which is distributed along a vast area of the country. One in 42 hares hunted showed histological lesions compatible with F. hepatica, and harbored a parasitic load of 6 liver flukes. This is the first report of this lagomorph acting as host of this helminth in the country and the third in South America. Yet, we found no previous reports regard the pathology of this fluke in the species. Thus, we emphasize the importance of more studies regarding introduced mammals which could be acting as reservoirs for this important fluke.


Resumo Fasciola hepatica é um parasita de origem europeia introduzido na América, de importância em saúde pública e animais de produção, especialmente em países com áreas endêmicas, como ocorre no Chile. Neste país, a pesquisa foi focada principalmente em ruminantes domésticos, com a fauna nativa e introduzida sendo amplamente ignorada. O objetivo desse trabalho é relatar a infecção por Fasciola hepatica em lebre europeia, espécie distribuída em vasta extensão do país. Uma em 42 lebres caçadas, mostrou lesões histológicas compatíveis com infestação por F. hepatica, alojando 6 vermes hepáticos. Este é o primeiro relato deste lagomorfo apresentando este tipo de parasitismo no Chile, o terceiro na América do Sul. No entanto, não encontramos relatos anteriores sobre a patologia deste acaso na espécie. Este relato salienta a importância de mais estudos considerando espécies introduzidas, as quais podem ser reservatórios desse importante parasita.

ABSTRACT

En el presente trabajo se registra la infección natural por Fasciola hepatica en un venado de cola blanca (Odocoileus virginianus) y en una taruca (Hippocamelus antisensis), ambos procedentes del departamento de Cusco. Los animales fueron remitidos al Instituto Veterinario (IVITA-Maranganí, FMV, UNMSM) por las autoridades del Servicio Nacional de Flora y Fauna (SERFOR, Sede Cusco). Durante la necropsia de los animales se colectaron seis trematodos de los conductos biliares, los cuales fueron preservados en etanol al 70%. Las observaciones morfológicas indicaron que se trataban de F. hepatica. Esto fue confirmado analizando el ADN mitocondrial de los parásitos amplificando parcialmente los genes citocromo c oxidasa subunidad 1 (cox1) y el NADH deshidrogenasa subunidad 1 (nad1). El análisis de estos genes tuvo una identidad mayor al 99% comparado con registros del banco de genes (GenBank). El presente estudio demuestra la presencia de F. hepatica en estos cérvidos, agregando así dos nuevos hospederos definitivos para el parásito.


Natural infection by Fasciola hepatica is recorded in a white-tailed deer (Odocoileus virginianus) and a taruca (Hippocamelus antisensis), both from the department of Cusco. Animals were remitted to the Veterinary Institute (IVITA-Maranganí, FMV, UNMSM) by the authorities of the National Service of Flora and Fauna (SERFOR, Cusco Headquarters). Six trematodes were collected from the bile ducts during the necropsy of the animals, and they were preserved in 70% ethanol. Morphological analysis indicated that they correspond to F. hepatica. This was confirmed by analyzing of the mitochondrial DNA of the parasites by partially amplifying the cytochrome c oxidase subunit 1 (cox1) and the NADH dehydrogenase subunit 1 (nad1) genes. Analysis of these genes had an identity greater than 99% compared to genes from GenBank. The present study demonstrates the occurrence of F. hepatica in these cervids, thus adding two new definitive hosts for the parasite.

Abstract Fascioliasis is a food-borne anthropozoonotic disease caused by Fasciola hepatica that affects multiple hosts, including humans. We herein report the first case of human fascioliasis in the state of Santa Catarina, Brazil. A 57-year-old female patient complaining of abdominal pain was admitted to the hospital for a clinical investigation. The diagnosis of F. hepatica was confirmed by ultrasound and indirect enzyme-linked immunosorbent assay. Authorities of the Northern coast of Santa Catarina were notified to investigate other cases and risk factors for contamination. The disease is also prevalent in cattle, which could pose as a potential route for infection.

Resumen La fasciolosis es una trematodosis que afecta fuertemente al ganado de Cuba. Se estudiaron 10 vaquerías del municipio más occidental de Cuba para determinar la prevalencia de esta parasitosis en el ganado y en los hospederos intermediarios. Se recolectaron y estudiaron muestras fecales de 455 vacas lecheras seleccionadas al azar. Las muestras fueron conservadas a -20 oC hasta su estudio. La detección de la infección por Fasciola hepatica fue realizada a partir de un método de Elisa denominado FasciDIG®. El análisis e interpretación de los resultados demostró que 146 muestras fueran positivas a F. hepatica (32,09 %). Todas las vaquerías presentaron animales infectados con rangos de prevalencia entre las vaquerías que oscilaron de 9,5 % al 84 %. También fue posible encontrar las dos especies de hospederos intermediarios de este parásito en Cuba, Galba cubensis y Pseudosuccinea columella. Debe prestarse atención a la prevalencia en bovinos encontrada en este estudio para prevenir la aparición de brotes humanos de esta enfermedad.


Abstract Fasciolosis is a foodborne trematodosis affecting many cattle farms in Cuba. Ten dairy farms in western Cuba were studied to assess the prevalence in both intermediate and definitive hosts. A single stool sample from 455 dairy cows was taken and studied. The animals were randomly selected and the samples were kept at -20oC until infection was determined. Detection of the Fasciola hepatica infection was carried out using a non-commercial sandwich-ELISA called FasciDIG®. Results showed that 146 samples were positive for F. hepatica (32.09%) with every farm testing positive for F. hepatica infection, while prevalence ranged from 9.5% to 84% among farms. It was also possible to detect the presence of the two species of intermediate hosts of this trematode in Cuba, Galba cubensis and Pseudosuccinea columella. Main attention must be given to bovine prevalence found in this study in order to prevent eventual human outbreaks of this disease.

Infection by Fasciola hepatica is an important cause of economic loss in cattle raised in Brazil and others countries. In slaughterhouses, the losses are mainly associated with condemnation of livers; however, it is believed that these losses can extend to diminished carcass yield and productivity throughout the life of the animals. Three slaughterhouses with a daily routine of approximately 15-20 cattle slaughters were monitored from July 2016 to November 2017. After sample collection, weighing, and liver assessment, statistical studies were conducted to determine disease prevalence and measurement of economic losses caused by fasciolosis through condemnation of the organ of choice of the parasite. Of the 210 livers assessed, 79 (37.6%) were condemned because of presence of F. hepatica, 87 (41.4%) were considered appropriate for human consumption, and 44 (20.9%) were considered inappropriate because of some other macroscopic lesions. The aim of this study was to estimate the prevalence of fasciolosis, correlate epidemiological and clinical-pathological aspects, and estimate the economic losses caused by the disease. It is estimated that the losses in southwestern Rio Grande do Sul state could reach R$ 36,943,532.60 (37.6%; 95% CI = 30,753,525.80 - 43,526,55.65) or US$ 11,437,626.20 (37.6%; 95% CI = 9,521,215.40 - 13,475,713.80) only because of condemnation of livers.(AU)


A infecção por Fasciola hepatica é uma importante causa de prejuízos econômicos para o setor pecuário no Brasil e em outros países. Em frigoríficos, as perdas estão relacionadas principalmente a condenação dos fígados, porém acredita-se que esses prejuízos possam se estender a perdas no rendimento de carcaça e durante a vida produtiva do animal. Durante o período de julho de 2016 a novembro de 2017 foram realizados acompanhamentos periódicos mensais em três frigoríficos, os quais, possuem uma rotina diária de aproximadamente 15-20 abates. Após a coleta de dados, pesagem e avaliação dos fígados foram realizados estudos estatísticos para determinação da prevalência e mensuração dos prejuízos econômicos causados por fasciolose pela condenação do órgão de eleição do parasito. Dos 210 fígados avaliados, 79 (37,6%) foram condenados devido a presença de F. hepatica, 87 (41,4%) foram considerados próprios para consumo humano e 44 (20,9%) impróprios por alguma outra alteração macroscópica. O objetivo deste estudo foi estimar a prevalência da fasciolose, e associar os aspectos epidemiológicos e clinico patológicos, além de estimar os prejuízos econômicos causados pela doença. Estima-se que as perdas na mesorregião Sudeste Rio-grandense possam chegar à R$ 36.943.532,60 (37,6%; IC.95% = 30.753.525,80-43.526.55,65) ou US$ 11.437.626,20 (37,6%; IC.95% = 9.521.215,40-13.475.713,80) somente devido a condenação de fígados.(AU)
